Any Alaskan student wishing to apply for the JoAnn LaCour Memorial Scholarship should prepare a scholarship folder including the following items:

1. The application included in this information packet.
2. A letter of application including: (A) Personal history-information about yourself and family, what  you like to do; plans for the future, general philosophy of life and how you feel about the nursing profession. (B) Plans for education- Why you want to pursue a nursing degree, which college you plan to attend and why you chose that college; any particular nursing fields that appeal or interest you. (C) Why you need this scholarship.
3. One photo- (Optional) The picture will be used for an announcement in your local newspaper if you are selected. Please indicate on the back of the photo if you want it returned.
4. Three letters of recommendation.  Suggested sources are Teacher, Employer or person who knows you well, Any other person of choice; preferably not a relative.
5. A High School or College transcript of grades and credit & ACT/SAT scores.

Place all information in a folder and mail directly to the Scholarship Committee. Folders will not be returned unless requested.  Mail to:
